In the section "How to get authenticated user in your apps?", the following instructions, mentioned in the readme file, does not work for Apache 2.4:

RewriteEngine On
RewriteCond %{LA-U:REMOTE_USER} (.+)
RewriteRule . - [E=RU:%1]
RequestHeader set X-Remote-User "%{RU}e" env=RU

Instead, the correct instruction, that works for me is:

RequestHeader set X-Remote-User expr=%{REMOTE_USER}
