New problem creating/editing site visits

I have newly been unable to create or edit any site visits that contain any nested actions. Here are the details:

• I can create a site visit with no nested actions, but when I try to add any, it fails.
• I try to edit an existing site visit containing multiple nested actions. The edits fail - it just brings me back to the Edit Site Visit page.
• I attempt to create a site visit with nested actions (but no instrument deployments or instrument calibrations). It does not create - brings me back to the Create Site Visit page with the info that I've entered.
• I attempt to create a site visit with a nested instrument deployment action. It does not create - brings me back to the Create Site Visit page. The results that I had added are now blank.
• I attempt to create a site visit with nested instrument calibration actions. It does not create - brings me back to the Create Site Visit page. I get an error that the IOV is not valid (it is valid). This has happened multiple times.

Many of these cases are addressed. However, when editing a site visit, I cannot add additional nested actions that are calibrations, instrument deployments, instrument retrievals, or equipment retrievals. I am able to create those actions as part of a site visit initially, but only if they are the only action - i.e., I am successful with adding a single calibration to a site visit, but am unable to add two calibrations to a site visit.

The problem has to do with Instrument Output Variables. This is the reason for re-designing using formsets, which is well on its way right now, and should resolve this problem.
