Error inserting Null in to "featureactionid' with multiple REsults

When submitting a Site Visit Form with multiple results for an action, an error is returned stating that Null cannot be inserted in to 'Featureactionid'

I've moved data saving back to the set_up_site_visit function, and will just be pulling data from forms as it was done before, to try and remedy this.
