Randomization form name is not displaying in the study edit modal

Overview

When attempting to edit a study, the randomization form name is not displayed if the study is already configured with a randomization form. It should display the form title similar to how it displays it for the termination form.

Background Knowledge

It looks like this might be an issue with the knockout binding for that modal. Randomization still works, this is just a rendering issue.

Steps to Reproduce

1. Go to a study view
2. Edit a view with an existing randomized study

Outcomes

Expected

• It should display the configured randomization form
  Actual
• It displays nothing
