Rework donation site import -- should have more fields, and give error when donation sites not imported.

Summary

Add contact, email and phone to the donation sites import

Why?

Not much point in only being able to import name and address when you have all this other info available to be entered

You will only see the import on a site with no donation sites
Sign in as [email protected]
click on community
click on donation sites
click on import donation sites
Here you can get a template and import a csv

To do

update the template to include contact, email, phone
add capability to import the full template

I'm not sure if we have any restrictions on donation site names (uniqueness in organization, perhaps), but if we do, an error should be displayed if not all the donation sites can be imported.

Criteria for completion

• behaviour as described above
• tests to support said behaviour

At first I thought lets get rid of the import, but several orgs have more than 30 sites
