feat: improve codelens support, restart on config changes

• add codelens support for #graphql delimeter
• restart codelens on graphql config changes
• restart content provider on config changes (fresh schema, etc)

TODO

• refactor to provide a shared config helper that provides config and endpoints for URL to share between providers
• add unit tests
