  1. omfdump: fixes and enhancements 

    - fixes decoding of FIXUP records
- memory mapped IO converted to ordinary file IO for compatibility
- ignores garbage after MODEND (produced by Digital Research RASM86)
- adds / enhances decoding of several record types
