Rewrite module

* Replace mdast with remark;
* Add support for `thematicBreak`;
* Refactor code style to `xo`;
* Replace `istanbul` with `nyc`;
* Replace `mocha` with `tape`;
* Add `.editorconfig`;
* Remove superfluous files;
* Remove duo, bower, component support;
* Remove distribution files from source;
* Add GH releases, npm deployment.
