Added ability to edit Input placeholder value in EditQuestionForm. Got copyFieldMappingRule working again. Started converting captureQuestionnaireResponsesListRetrieveData to captureAnswerListRetrieveData. Added helper functions getAnswerToQuestion, getAnswerValueToQuestion, getQuestionById and getQuestionsForQuestionnaire. Moved AnswerQuestionsForm.jsx to AnswerQuestions.jsx and got it working. #83
